Health at Home LLC is planned as a Delaware limited liability company operating in the U.S. home healthcare and nursing services industry. Its initial focus is Central Florida, with a phased strategy for broader geographic expansion.

The company's mission is to reduce preventable hospital readmissions, enhance patient quality of life, and expand access to high-quality home healthcare—especially for older adults, post-operative patients, people with mobility limitations, and individuals managing chronic conditions.

Planned nursing visits for post-hospital and post-operative patients, including vital-sign monitoring, medication administration, wound management, infusion therapy, catheter care, and clinical follow-up within the authorized scope of practice.

Nurse-led monitoring and education for people managing conditions such as diabetes, hypertension, COPD, and heart failure, with an emphasis on adherence, prevention, and early recognition of complications.

Naiane Larissa Sousa Cortes

A nursing professional educated in Brazil with postgraduate specialization in Obstetrics and Intensive Care / Emergency and Urgent Care, and experience across hospital environments including intensive care, emergency, surgical, medical, and wound-care settings.

Her clinical and operational background includes patient-safety initiatives, nursing-team education, emergency-response training, preventive health education, and participation in skin-care and wound-management protocols. Health at Home LLC is designed to translate that frontline experience into standardized, patient-centered home healthcare operations.
